Role of imaging in Obstructive Jaundice.

The diagnostic value of ultrasonography and percutaneous transhepatic cholangiography (PTC) was compared in 39 patients with obstructive jaundice. The final diagnosis was obtained by surgical findings. Transhepatic cholangiography diagnosed obstruction and its level in all patients whereas ultrasonography failed to find obstruction in one patient and was unable to determine the level of obstruction in 6 patients. The cause of obstruction was correctly assessed in 35 patients by transhepatic cholangiography and in 31 by ultrasonography. In five patients with obstruction caused by common duct calculi, ultrasonography only diagnosed three. Ultrasonography is a reliable tool for the diagnosis of obstructive jaundice and in most cases for localization of the level of obstruction. However, diagnosis of the cause of obstruction and of its precise topography requires direct cholangiography.
