Intrarenal and intravesical Color Doppler Sonography in patients with acute renal colic.

Objectives: To examine intrarenal arterial doppler characteristics and ureterovesical jets by color doppler sonography in patients with acute renal colic. Design: Cross-sectional study. Place & Duration of Study: The study was conducted at the Department of Radiology, PNS Shifa Karachi from February 2002 to August 2002 over a period of 7 months. Patients And Methods: The study was performed on 100 patients with renal colic due to suspected renal or ureteric calculi. The renal obstruction was excluded via color duplex doppler sonography with 3.5 MHz probe by means of resistive index (RI) of the intrarenal arteries and uretrerovesical jets as seen in full urinary bladder. Results: The mean RI for obstructed kidneys was 0.77 (SD=0.04), which was significantly higher (p value < 0.05) than the mean RI of the contralateral normal kidneys 0.64 (SD=0.06). There was also significant difference in the RI of the intrarenal arteries of obstructed kidneys and normally functioning contralateral kidneys 0.083 (SD =0.0071) p<0.05. RI greater than 0.7 or an inter-renal difference in RI greater than 0.08 compared with the normal kidney proved reliable cut-off values to diagnose renal obstruction. Patients with renal obstruction had either absent or continuous low level ureteric jets. Conclusion: Changes in resistive index and ureterovesical jets, as measured by color duplex doppler sonography, can contribute significantly to the diagnosis of. unilateral renal obstruction.
