Tubal Ectopic Pregnancy; Critical evaluation of presentation & management.

OBJECTIVE: To discusses the clinical presentation and explore the outcome of management of tubal ectopic pregnancy. SETTING: Department of Obstetrics and Gynecology, Nishtar Hospital Multan. PERIOD: February 1995 to July 1996. PATIENTS, METHODS & RESULTS: Thirty patients were studied prospectively. Detailed history was obtained along with clinical examination and investigations especially ultrasonography. Laparoscopy was done if the diagnosis was uncertain. Lower abdominal pain and amenorrhoea were almost constant features of ectopic pregnancy. Tubal ectopic was found in 97.7% cases and ovarian ectopic in 3.3% cases. Salpingostomy was required in 63.3% cases. Milking salpingotomy and salpingo-oophorectomy was performed in 10% and 3.5% cases respectively. Removal of products of conception along with blood clots and peritoneal toilet was performed in 13.3% cases. CONCLUSION: Management of tubal ectopic pregnancy by laparotomy is more practicable in socio-economic setup of developing countries like ours, where patients present late.
