Management of Acute Upper Gastrointestinal Hemorrhage.

Our experience of acute upper gastro intestinal hemorrhage in past 5 years at Shifa International Hospital is reviewed. We have treated 447 patients, out of these varices 156 had bleeding from esophageal varices with a 35% incidence. In these patients, 20% had duodenal ulcer and rest of the patient had gastritis, gastric carcinoma, gastric ulcers, esophagitis and esophageal carcinoma as causes for bleeding. Endocscopic examination was performed within twelve hours in all the patients and endoscopic therapy was performed, where necessary. Patients required 0-10 blood transfusions. Ten patients were sent for surgery. Hospital stay ranged from 2-10 days. Mortality for the patients with variceal bleeding was 31% and for non variceal patients, it was 2% NSAID use was about 12%. Management of bleeding involves urgent upper gastrointestinal endoscopy with proper endoscopic management. The mortality of the patients with non variceal bleeding was in acceptable range. However, mortality for the variceal bleeding remains high.
