Many patients present to emergency rooms with altered behavior. The emergency room is a busy place and sometimes patients are assigned to the psychiatric ward without adequate medical clearance. It is important to differentiate between purely psychiatric illnesses, from medical emergencies presenting with psychiatric symptoms. When the patient is unresponsive, this can be achieved by taking a careful history from the person who has spent the most time with the patient during the presenting illness. Also, paying attention to the possibility of masked signs and symptoms due to prior ‘blind’ symptomatic treatment and re-evaluation if there is no response to recommended doses avoids missed diagnoses.
