Prevention of recurrence with Intra Operative Mitomycin C - A prospective trial in Primary Pterygium Surgery.

Pterygium recurrence following excision is a fairly serious problem and is responsible for disability and loss of work for quite some time, particularly so in high-risk population groups including personnel in Pakistan Armed Forces. Our randomized, placebo controlled, prospective, interventional trial was aimed to study the effect of intra-operative mitomycin C (MMC) in reducing recurrence following excision of primary Pterygium. We used 0.04% MMC as a single intra-operative application and included 100 eyes of 92 patients with primary Pterygium in our study. Excision was performed using bare sclera technique under local anesthesia with 2% xylocaine. Patients were randomized into two groups, a control group, treated with normal saline solution and MMC treated group, treated with intraoperative MMC (0.04%). Minimum follow-up was carried out for six months. Intraoperative MMC (0.04%) has been found as a useful adjunct chemotherapy for prevention of Pterygium recurrence in our study. No serious complications were seen, however, they were slightly more frequent as compared with simple excision. Therefore, use of 0.04% MMC as a single intra-operative application is strongly recommended as an adjunct chemotherapy in surgery for primary Pterygium particularly so in high-risk population groups including serving soldiers.
