Riedel`s Thyroiditis.

A 44 years old female presented with a large goiter and increasing exertional dyspnea and dysphagia. Clinically, she had signs and symptoms of hypothyroidism. Biochemically, she has hypothyroidism, and radioisotope scan showed multiple areas of icreased and reduced tracer concentration in both the lobes of enlarged thyroid gland. Background activity appeared reduced. X-ray of neck showed a large soft tissue mass in anterior part of the neck extending downwards behind the sternum and causing tracheal narrowing. Intraoperatively, she had adherent thyroid gland and appeared fibrosed. Postoperatively, she developed hypocalcemia which was managed and patient responded well. Histopathology of the thyroid gland revealed atrophic follicles surrounded by marked chronic inflammation and dense fibrosis.
