Atrial fibrillation and other arrhythmias in patients early after Coronary Artery Bypass Grafting (CABG) Surgery.

Objective: To find out the prevalence of atrial fibrillation and other arrhythmias in patients undergoing CABG surgery. Place and Duration of Study: This study was conducted at AFIC/NIHD, Rawalpindi. Patients operated upon during Sep/Oct. 2001 at the hospital were included in the study. Subjects and Methods: All patients undergoing CABG surgery who were in sinus rhythm prior to surgery were enrolled. Patients in atrial fibrillation and/or with permanent pacemaker prior to surgery were excluded from the study. The variables associated with the occurrence of arrhythmias were assessed and evaluated. The patients were monitored for initial 48 hours in ICU and later on in surgical wards. Results: Of 42 patients studied, 06 (13.3%) developed transient AF requiring pharmacological intervention; 01 (2.2%) patient developed SVT, while 01 (2.2%) patient experienced VT post operatively. Maximum arrhythmias (07) were seen in first 48 hours after surgery. All patients who developed AF were male, mean age 67.5±12.6 years (range 48 – 84), and all had TVCAD with normal LV ejection fraction. Patient developing ventricular tachycardia was male of 45 years age, with TVCAD and normal LV EF. The patient of SVT was female of 45 years and with TVCAD and normal LVEF. Conclusion: The commonest post-CABG arrhythmia was AF. Male sex, advancing age and more aggressive coronary artery disease were the strongest variables associated with increased occurrence of AF.
