Prevalence and pattern of ovarian lesions.

Background: Ovarian pathology is one of the common lesions seen in female genital tract. Ovarian cancer is the 5th most common malignancy amongst women and comprises 23% of all gynaecological malignancies. Place and Duration: The study was carried out in the Histopathology department, Pakistan Institute of Medical Sciences, Islamabad, from July 2001 to December 2004. Objective: To study the relative frequency and histological pattern of ovarian lesions. Materials and Methods: The surgical specimens of ovarian Pathology received in the department were included in the study. The representative areas taken during gross examination and submitted for examination. The specimens were fixed in 10% formalin and routinely processed in automatic tissue processor. The sections were then cut by microtome and stained with Hematoxylin and Eosin stain. Results: A total of 319 of ovarian lesions were studied during this period. The benign lesions were 100 (31.4%), benign tumours were 148 (46.4%) and malignant tumours were 71 (22.2%). The benign lesions comprised of hemorrhagic cysts (80%) and endometriosis (20%). The main bulk of ovarian tumours (219 cases) comprised surface epithelial tumours (69.4%). There were 20.5% germ cell tumours and 10.1% sex cord stromal tumours. The benign category comprised serous cyst adenoma (50.7%), mucinous cystadenoma (16.9%), mature cystic teratoma (18.9%), struma ovarii (1.4%), fibroma (8.8%) and thecoma (3.3%). In the malignant group, there were serous cystadenocarcinoma (35.2%), mucinous cystadenocarcinoma (28.2%), endometrioid carcinoma (5.6%) malignant Brenner tumor (4.2%), yolk sac tumour (8.5%), dysgerminoma (7.0), embryonal carcinoma (2.8%), immature teratoma (2.8%), granulosa cell tumor (4.3%) and poorly differentiated stromal tumour (1.4%). Conclusion: Amongst the ovarian lesions, the surface epithelial tumours comprised main bulk of tumours. In malignant surface epithelial tumours, serous Adenocarcinoma was the commonest, followed by mucinous adenocarcinoma.
