The role of Doppler Ultrasound in the management of high-risk Pregnancies: A PIMS experience.

Back ground: Doppler velocimetery of umbilical artery should be done as a routine in high risk pregnancies Objective: To assess the role of umbilical artery Doppler examination in the management of high-risk pregnancies. Study Design: Prospective, descriptive and interventional. Setting: Department of gynaecology and obstetrics, Unit II MCH, PIMS, Islamabad. Patients and Methods Population: Fifty four women with singleton pregnancies at high risk of IUGR delivered between June 2004 to December 2004. Main Outcomes Measures: Mode of delivery, APGAR score, birth weight and NICU Admissions. Results: Mean maternal age was lower in abnormal Doppler compared to normal Doppler group (25.8 vs 34.8), normal Doppler group showed 6 (31%) Emergency C sections performed compared to 14 (38%) in abnormal Doppler group, 5 patients 26.3% in normal Doppler group and 12 patients 33% in abnormal Doppler group delivered vaginally. At 5 min of birth Apgar scores of 5-10 were seen in all the babies belonging to mothers of normal Doppler group and in the other group 95% babies showed the same score. NICU admissions were 15% in the normal Doppler group and in the other group they were 22%. Conclusion: The Doppler velocimetery result was beneficial in those patients who were having SGA fetuses with abnormal UA indices.
