The B-Lynch suture technique for the control of massive PPH because of uterine atony.

Objective: To determine the effectiveness of B-Lynch suture in refractory post partum haemorrhage. Design: Prospective observational single centre study. Duration: July 2000 to July 2003. Sample: 29 patients with refractory post partum hemorrhage out of which 17 underwent B-Lynch procedure Methods: This hospital based 3 years study was carried out to find the effectiveness of B-Lynch suture in controlling primary PPH due to refractory uterine atomy. Results: There were a total of 189 cases of primary PPH. They were because of retained placenta, uterine atony, and genital tract trauma etc. Out of these, 29 patients were managed surgically (in addition to medical methods). Out of cases of PPH, retained products of conception and traumatic causes were excluded. B-Lynch suture was applied in 17 patients, 12 were applied at cesarean section and 5 were a sequel to vaginal delivery. In two patients uterine artery and internal iliac artery ligation were also performed. The procedure was successful in 14 patients. Failure of B-Lynch was more with placenta praevia, previous C-section and DIC. B-Lynch suture is particularly useful because of its simplicity of application, potentially life saving and useful is preserving fertility. It is a good alternative to hysterectomy.
