A rapidly mounting up interest in the subspecialty of urogynaecology by clinicians as well as pharmaceutical companies has led to a number of interesting techniques in the management of female urinary incontinence.urodynamic investigations is now considered mandatory before attempting any form of surgical intervention for stress urinary incontinence in the UK health care setting.For urethro vesical /proximal urethral dysfunctionleading to stress leak, a variety of surgical operations have been performed for over a century. we have experience with insertion of suburethral slings of different varieties in selected patients with urodynamic stress incontinence proven by urodynamic investigation of cystmetrogram.
The transobturator tape insertion employs either inside out or outside in approach for the placement of a polypropylene mesh suburethrally passing through the obturator foramen.it works as a hammock around proximal urethra and urethrovesical junction to restore anatomical configuration and maintain continence by increasing urethral pressure during episodes of raised intraabdominal pressures like coughing,sneezing, bending,walking,lifting etc.transobturator tape is a minimally invasive proceedure with more safety and less complications compared to traditional open proceedures like Burch colposuspension.
recent evidence has led us more confidence regarding its success in restoring continence and improvement in quality of life of patients with stress incontinence giving them more freedom in their social and physical functioning.

How many of these procedures have you done and what's your success rate?

apologies for delayed reply.
the procedure is being done under NICE guidance arrangements in Uk presently. i have personally done about 40 and have come up with > 80% clinical success. i have also conducted a prospective study of subjective outcome and the results were 92% patient satisfaction with the technique.

furthermore, routine intraoperative cystoscopy is not required with obturator tapes. the technique is preferred over TVT esp in pts previously operated for incontinence or in obese and elderly.good outcomes are reported in short to intermediate term. further data is awaited from large trials.
